Foundations Early Learning Center values the families, team members, users and visitors of this website. We are committed to protecting the information you provide by coming to the site, and that you submit through enrollment or employment forms.
The information your computer or Internet subscription provides us when you visit this site is used strictly for evaluating site traffic numbers and patterns. We do NOT link this general IP information to any personal information you may provide by filling out enrollment and/or employment forms. The IP information is simply used to evaluate our site’s performance, and is for internal use only.
Any personal information you provide such as your name, phone number, email address, home address or any information about your child will only be used internally, and by the appropriate employees of Foundations Early Learning Center. In an effort to enhance customer service, we do reserve the right to use contact information we gather from enrollment or employment forms to provide you additional information about our services and respond to inquiries.
Visits to this site and submission of information via enrollment or employment forms constitutes your acceptance of these policies and the limited use of information as described herein. Any and all information you provide is strictly for internal use by Foundations Early Learning Center, and will not be shared, rented, bartered for or sold to any third party. To file a program discrimination complaint, a Complainant should complete a Form AD-3027, USDA Program Discrimination Complaint Form which can be obtained online at: https://www.usda.gov/sites/default/files/documents/USDA-OASCR%20P-Complaint-Form-0508-0002-508-11-28-17Fax2Mail.pdf, from any USDA office, by calling (866) 632-9992, or by writing a letter addressed to USDA. The letter must contain the complainant’s name, address, telephone number, and a written description of the alleged discriminatory action in sufficient detail to inform the Assistant Secretary for Civil Rights (ASCR) about the nature and date of an alleged civil rights violation. The completed AD-3027 form or letter must be submitted to USDA by:
